Below you will find forms necessary for success in Disability Access Services.
Student Checklist
Our office has prepared a Student Checklist
to help new and returning students remember the steps they need to
complete to arrange accommodations. It is strongly recommended that you
follow and use the checklist to help you become familiar with Disability
Access Services policies and procedures.
DAS Application:
If you are not currently registered with Disability Access Services and would like to request services then the first step is to complete the DAS Application. You can download and save the application (it works with most word processors). From there you will be able to complete and print the form. At this time it is not possible to submit this form online.
Registration Form:
If you are currently registered with Disability Access Services, and you are returning, please complete the DAS Registration Form. This form will alert DAS that you are returning for the upcoming semester, and we can have your paperwork ready for you.
DAS Out-of-Class Testing Form:
Once you are registered with Disability Access Services you may need to
take your exams in a distraction free environment or
utilize special equipment when taking your exams. Students registered with DAS have the option to take their exams at the Campus Testing Center (412 SSB) or in the DAS office. Disability Access
Services has three 'testing rooms' offering distraction free spaces and
access to special equipment.
For these circumstances you will need the DAS Adaptive Testing Form. Students complete a portion of this form, and the instructors complete the other portion. This form serves to notify instructors that a student intends to take his/her exam in an alternate location with accommodations. The form also notifies DAS personnell that a student is intending to take an exam and needs to schedule an appointment (to ensure that there is space available).
