Provincial House Chapel Wedding Date Request Application

Please click on the link below and fill out all fields of this form to the best of your knowledge. Please also note the following:

  • Please call the Coordinator and confirm the availability of the wedding date(s) you have in mind prior to filling out this form.
  • Due to the popularity of the chapel, reservations are made on a first come, first serve basis. Our office will hold your requested date application for 48 hours without the $280 prepayment. Please note that our receipt of an application does not guarantee the requested date.
  • Following receipt of your application, an email will be sent to the email address you provide. This email will confirm receipt of application as well as the expiration date and time of your 48 hour hold. Please be sure to check your email for this notification!
  • Acceptable forms of deposit payment: cash, check, or money orders made payable to University of Missouri - St. Louis. Credit card payments can also be accepted in person or over the phone.
  • Upon our receipt of your application and $280 prepayment of the facility fee, a confirmation package will be mailed to you. Your wedding is considered booked only when you receive a written confirmation package from Conference and Event Services.
  • All weddings must be booked at least 60 days in advance.
