STEM 6 Month Validation Report

Please complete this form every six months if you are an F-1 student currently on a 17-month OPT Extension. This form must be completed within 10 calendar days of each six month period during your extension. You must continue to submit this report until the end of your extension or your F-1 status whichever occurs first.

Example: If your start date was 2/01/2009 you will need to submit a report within 10 days of 7/31/2009 and 1/27/2010.

Additionally, we are asking students to complete this validation report when they receive their EAD showing the extended OPT period.
