The School of Optometry has been identified to contain health information records* containing one or more of the following items that can be identified to a specific individual, their relatives, their employer, and/or members of their household:

| 12 | Vehicle identifiers and serial numbers, including license plate numbers |
| 13 | Device identifiers and serial numbers |
| 14 | Web universal resource locators (internet URLs) |
| 15 | Internet protocol (IP) address numbers |
| 16 | Biometric identifiers, including finger and voice prints |
| 17 | Medical diagnosis |
| 18 | Full face photographic images and any comparable images |
| 19 | Any other unique identifying number, characteristic, or code |
*Health information records are those created or received related to the past, present, or future physical or mental health or condition of an individual; the provision of health care to an individual; or the past, present, or future payment for the provision of health care to an individual.
