Telephone Service Online Work Orders: Telephone Services has automated the work order process.  Please use the link above to fill out and submit work orders for Installs, Disconnects, Upgrades or Moves.

INSTRUCTIONS: You will sign in with your SSO ID and password and select type of work order.  Install, Upgrade and Move requires a MoCode.  The Disconnect work order requires the extension.

You are the “requestor”, you may be the MoCode Authorizer and/or the MoCode owner (designated in the Telephone Services billing application).  As such, you may receive up to 3 confirmation emails regarding the work order.  Selection of a MoCode Authorizer is required.  If the information in the email is not correct, please reply to the email with explanation.  The work order will be cancelled and you will be asked to resubmit the work order.

Optional fields include selecting a “preferred date” for completion of the work order and a “comments” field for additional information.

At the bottom on the screen, you will see a “flashing bar” titled (depending on type of work order) ADD A NEW INSTALLATION, ANOTHER DISCONNECT, UPGRADE EQUIPMENT, or MOVE EQUIPMENT.  Click on the “flashing bar” and another window will appear.  Input the appropriate information.

  • Installs – Select equipment type, check “long distance” if desired; and check yes or no for voicemail (this is a required field). 
  • Disconnects – you can only disconnect phones charged to the same MoCode on a work order.  If extension does not appear in the list, please contact Telephone Services at x6500.
  • Upgrades – Select new equipment type and the extension. 
  • Moves – Select extension, current location automatically appears, enter new location.

On all work orders, please select FINISH on the “input window”.  Repeat entries as necessary by clicking on the “flashing bar” again.  An entry can be edited or deleted.

When completed, select SUBMIT.  If you change your mind, please select CANCEL and “logout”.  In either case, an informational message will appear.

Telephone Services Billing Change Form: The Billing Change form is available on-line. Please select, fill out and print. After you have obtained an authorized signature, send to Telephone Services (fax to X6019 or mail to 113 Lucas Hall).

Installation of New Phone Jacks: To complete an install or a move, installation of new phone jack(s) may be required and billed for time and materials (minimum cost of $250 per cable run). This charge is in addition to the $145 install fee or the $125 move fee as noted below.

Please allow time for jack installation when planning any telephone line moves or new installations. For new construction, if data cable is also required, please contact Facilities at x6320 to place a request for both phone and data cable installation before submitting the move or install work order. Pricing Structures: Pricing structures are dependent on the type of payment account used to pay for services.

  • "In the Plan" accounts are paid by rate dollars (general revenue allocations).
  • "Out of the Plan" accounts are paid by non-rate dollars.
  • "Grant" accounts are paid by grant monies.

Caller ID: Please note that Caller ID boxes are no longer supported by Telephone Services. If Caller ID is desired, an equipment upgrade to a "Display" phone will be required.
