General Purpose Answer Sheets

The Optical Scanning area stocks 3 different types of general purpose answer sheets that can be used for exams, course evaluations, and surveys. The forms are available in room 214A Lucas Hall. They are packaged 500 in a box for $50.00 ($.10 each). On-campus users need to supply a MOcode when purchasing GPAS

Not sure which form is best for your application? A description of each sheet is below. To see the front side of the form, click on the form color in the form description. Depending on your viewer, you may have to scroll to the right and down in order to see all the fields on the form.

Each form type has grids for NAME, ID NUMBER, SPECIAL CODE, SEX, GRADE, and BIRTH DATE. Any and all of the grids or questions can be used.

Blue forms have 200 questions, each with 5 possible responses. The response positions are labeled 1-5 within the circles and A-E above the circles.

Red forms have 240 questions, each with 5 possible responses. The response positions are labeled T & F in the first two circles and A-E above the circles.

Green forms have 120 questions, each with 10 possible responses. The response positions are labeled 0-9 within in the circles and A-J above the circles.
