  Use this to create "quick and dirty" instant forms. This javascript utility consists of a form that you will fill out. The results will be functioning HTML and javascript code that you can use immediately.

  Refer to two sets of source documentation for Jerry Siegel's FormServlet. This will allow you to create your own forms with more flexibility than is allowed by FormServlet Formmaker (but you'll have to do your homeword carefully!).
