Performance Self-Appraisal Instructions

At the bottom of this page is the link to Performance Self-Appraisal form for your position – management or staff.

Use the form to review your own performance over the past year and bring the completed form with you to your Performance Appraisal meeting with your supervisor. This self-appraisal should not only enhance your participation in the appraisal process, but ultimately play a significant role in your personal development and your contribution to the growth of the department.

To prepare for your Performance Appraisal meeting:

  • Review your job description to make sure you understand your job expectations.
  • Review your department's mission and goals.
  • Think about your relationship and communication with your supervisor and your co-workers.
  • Think about what you have done to improve your department and the university.
  • Think about ideas you have for improving your department and the university.

Although the Performance Appraisal form for the most part is self-explanatory, please follow the instructions below when completing the form:

Determine the rating

On factors where expectations are not defined for you, consider beforehand the specific results to be achieved, specific tasks you must perform or the specific behaviors you must demonstrate in order to meet the broader expectations described on the form. Compare your performance to those tasks and behaviors, citing specific examples whenever possible.

The following definitions may help you decide how to rate yourself:

(3) Exceeds Expectations

  • Consistently went beyond goals or responsibilities
  • Performed exceptionally high quality work
  • Built upon ideas provided by supervisor
  • Suggested innovations and alternatives on own initiative
  • Required minimal supervision
  • Willingly took on additional responsibilities
  • Worked extra hours on own volition
  • Volunteered to help other employees
  • Provided leadership
  • Displayed enthusiasm for the job and the university's and department's goals

(2) Meets Expectations

  • Good performer, usually accomplished goals or responsibilities
  • Did what was asked or required without expressing disagreement or resentment
  • Weaknesses were offset by strengths
  • Dependable
  • Required some supervision but did not require unusual supervisor or co-worker guidance, support or intervention
  • Supported university's and department's goals
  • Sometimes took initiative or looked for a better way to do the job

(1) Below Expectations

  • Needs improvement, possibly due to newness in position
  • Often did not accomplish goals or responsibilities
  • Performance was not good enough to warrant recognition
  • Required more than normal guidance, support or intervention to meet expectations
  • It would materially benefit the department if this area were improved
