Disability Verification Criteria
- A clear statement of the DSM-IV diagnosis, including pertinent history.
- A description of present symptoms, fluctuating conditions or symptoms,
and prognosis.
- Current documentation (the age of acceptable documentation is dependent
upon the disabling condition).
- A summary of how the disability affects one or more major life functions.
- A summary of assessment procedures used to make the diagnosis.
- Medical information to be considered in a college environment, including
medication needs.
- Suggestions of reasonable accommodations supported by the diagnosis
