Physical and/or Health Related Disability
Disability Verification Criteria
Documentation of your physical or health related disability should include the following:
- A clear statement of the DSM-IV or ICD diagnosis, including pertinent
history.
- Current documentation (the age of acceptable documentation is dependent
upon the disabling condition).
- A summary of how the disability affects one or more major life functions.
- A summary of assessment procedures, including all scores used to make
the diagnosis.
- A description of present symptoms, fluctuating conditions or symptoms,
and prognosis.
- Other medical information to consider in a post-secondary educational
environment. This includes current medication needs.
- Suggestions of reasonable accommodations which must be supported by
the diagnosis.
- A description of personal care concerns
