Physical and/or Health Related Disability

Disability Verification Criteria

Documentation of your physical or health related disability should include the following:

  1. A clear statement of the DSM-IV or ICD diagnosis, including pertinent history.

  2. Current documentation (the age of acceptable documentation is dependent upon the disabling condition).

  3. A summary of how the disability affects one or more major life functions.

  4. A summary of assessment procedures, including all scores used to make the diagnosis.

  5. A description of present symptoms, fluctuating conditions or symptoms, and prognosis.

  6. Other medical information to consider in a post-secondary educational environment. This includes current medication needs.

  7. Suggestions of reasonable accommodations which must be supported by the diagnosis.

  8. A description of personal care concerns
