What is documentation of disability?

Documentation of a disability is written proof that a disability exists. It is necessary to provide documentation as justification for appropriate accommodations.

Documentation must be supplied by a qualified professional and must address the criteria outlined for the following disorders:

Students requesting accommodations must present written documentation of their disability that meets these criteria. The documentation must be on professional letterhead, signed, then mailed or faxed by the qualified professional directly to the Disability Access Services Office. Disability Access Services reserves the right to refuse documentation that is hand-carried to the office.

Additionally, verification forms are available in the Disability Access Services Office. A Release of Information Form will need to be signed by the student so that we can obtain written documentation from a qualified professional. Again, Documentation Verification forms must be mailed or faxed directly to Disability Access Services Office.

Special accommodations for national tests given through Educational Testing Service (ETS), such as LSAT, GRE, PRAXIS, CPA, must be requested through the Educational Testing Service using their documentation guidelines.
