Head Injury or Acquired Brain Injury

Disability Verification Criteria

Current medical/psychological information to verify the presence of a head injury/acquired brain injury is required to document and support a student's need for special classroom accommodations. The following list of criteria is provided for your information as a guideline to provide appropriate documentation:

  1. A current statement of the head injury or acquired brain injury, including probable site(s) of lesion(s).

  2. A summary of how the disability affects one or more major life functions.

  3. A summary of assessment procedures and evaluation instruments used to make the diagnosis.

  4. A narrative summary of cognitive and achievement measures and evaluation results, including standardized scores, used to make the diagnosis.

  5. A summary of present residual symptoms that meet the criteria for diagnosis.

  6. Medical information to be considered in a college environment, including medication needs.

  7. Suggestions of reasonable accommodations supported by the diagnosis.

  8. A description of personal care concerns.
