Attention Deficit Hyperactivity Disorder

Disability Verification Criteria

  1. A clear statement of the DSM-IV or ICD diagnosis and a description of supporting past and present symptoms.

  2. Current documentation (the age of acceptable documentation is dependent upon the disabling condition).

  3. A summary of how the disability affects one or more major life functions.

  4. A narrative summary of assessment procedures, including all scores used to make the diagnosis.

  5. A description of present symptoms, fluctuating conditions/symptoms, and prognosis.

  6. Other medical information to consider in a post-secondary educational enviornment, including medication needs.

  7. Suggestions of reasonable accommodations suported by the diagnosis.
