Disability Verification Criteria
- A clear statement of the DSM-IV or ICD diagnosis and a description of
supporting past and present symptoms.
- Current documentation (the age of acceptable documentation is dependent
upon the disabling condition).
- A summary of how the disability affects one or more major life functions.
- A narrative summary of assessment procedures, including all scores used
to make the diagnosis.
- A description of present symptoms, fluctuating conditions/symptoms,
and prognosis.
- Other medical information to consider in a post-secondary educational
enviornment, including medication needs.
- Suggestions of reasonable accommodations suported by the diagnosis.
