Submitting Test Transmittal Forms
You may submit your Test Transmittal Form along with your examto the Campus Testing Center in one of two ways:
- To print out your test transmittal form, complete it by hand and mail it along with your exam to the Campus Testing Center via campus mail, click here.
- To complete the test transmittal form on line, save it as a word file on your computer and send it along with your exam as attachments via email, click here.
