1. Change the title of a program
    Complete a Program Change Form and check the line that says, "Title change only." Include a rationale.
  2. Add an emphasis area or minor to an existing program
    Complete a Program Change Form and check the line that says, "Option added." Write a rationale and submit the curriculum for the emphasis area.
  3. Embed a graduate certificate program within an existing program
    Complete a Program Change Form and check the line that says, "Addition of certificate." Write a rationale and submit the curriculum for the certificate.
  4. Combine existing programs
    Complete a Program Change Form and check the line that says "Combination program." Write a rationale and submit the new curriculum.
  5. Delete an emphasis area or minor
    Complete a Program Change Form and check the line that says, "Delete option." Include a rationale.
  6. Delete a program (not recommended unless accompanied by a substituted new program)
    Complete a Program Change Form and check the line that says, "Delete program." Include a rationale.
  7. Make a program inactive
    (instead of deleting programs, it is advisable to pull them off of active status.)
    Complete a Program Change Form and check the line that says, Program placed on Inactive Status. Include a rationale.
