Conference pre-registration deadline is September 26, 2005.
Registration must be received in the office by that date, with full payment, in order to receive the pre-registration price.

All persons attending--including program participants and exhibitors--are required to register.  Please pre-register for all tours and meals, as space is limited.  Pre-registration packets will not be mailed.

Refund Policy
Refunds less a $15.00 handling fee will be given for cancellations received by September 26, 2005.  No refunds can be given for cancellations after that date. 

Membership in the Western History Association is not required to attend the conference, but is encouraged.

Some notes on the fill-in form:
Acrobat Reader may not allow you to save the form with the data you have typed in so you will have to print a hard copy and then mail or fax it.  If you have the ability to save this file using other Acrobat programs, you can email the file to us as an attachment.  If you have the ability to print to a PDF (select printer: Adobe PDF), you can create a file by doing so and send that file as an attachment.  (See your program's Help files for information.)
